Roosevelt Jazz Booster Application

To go Back Stage to view the Jazz Program's confidential information, you must be a member of the Roosevelt Jazz Boosters.  When your student was selected for one of the jazz groups, rosters of families were prepared and these will be used to authenticate the status of these Boosters with the program for access purposes.  If you are on a jazz group roster for the current year, please proceed directly to the Back Stage login page and register.  If you are an alumni performer or family member please fill out the following form to assist the Webmaster in authenticating your status with the Program. After identity verification you will receive an email indicating that you may proceed to the automated registration procedure where you can create a Username and Password.  Thank you for supporting Roosevelt Jazz.
